HUBUNGAN ANTARA EKSPRESI BAX, BCL-2, DAN RASIO BAX/BCL-2 DENGAN RESPONS KEMOTERAPI FASE INDUKSI PADA LEUKEMIA MIELOID AKUT ANAK

Leukemia mieloid akut merupakan keganasan terbanyak pada anak. Pengobatan utama leukemia anak adalah kemoterapi. Target obat kemoterapi adalah kematian sel terprogram atau apoptosis. Apoptosis pada kemoterapi merupakan apoptosis jalur intrinsik yang dikendalikan oleh famili b cell lymphoma-2 pada membran mitokondria yang terdiri dari agen proapoptosis dan antiapoptosis. Sebagai agen proapotosis adalah protein Bax dan agen antiapoptosis adalah protein Bcl-2. Adanya ketidakseimbangan pro dan antiapoptosis mempengaruhi prognosis terapi. Penelitian ini ingin mengetahui hubungan ekspresi Bax dan Bcl-2 maupun rasio Bax/Bcl 2 terhadap respons kemoterapi yaitu hasil bone marrow puncture (BMP) yang dijadikan marker prognosis setelah subjek mendapat kemoterapi fase induksi dalam 12 minggu pengobatan. Desain penelitian adalah experimental pre-post test, subjek merupakan pasien leukemia mieloid akut yang mendapat pengobatan kemoterapi fase induksi di RS.Dr.Saiful Anwar Malang menggunakan protokol kemoterapi revisi 2015 dengan rentang usia 1-14 tahun selama periode bulan Mei-September 2018. Hasil  menunjukkan hubungan terbalik yang tidak signifikan antara peningkatan Bax dengan hasil BMP, semakin tinggi Bax menunnjukkan hasil BMP tidak remisi (p = 0,893). Ekspresi Bcl-2 dengan hasil BMP menunjukkan hubungan tidak signifikan dengan korelasi negatif, semakin tinggi Bcl-2 pasca kemoterapi menunjukkan hasil BMP tidak remisi (p = 0,586), sedangkan rasio Bax/Bcl-2 setelah kemoterapi berbeda tidak signifikan (p= 0,594), dan hubungan rasio Bax/Bcl-2 dengan hasil BMP adalah tidak signifikan dengan korelasi positif semakin tinggi rasio semakin tidak remisi (p = 0,104). Kesimpulannya, peningkatan Bax tidak berhubungan signifikan dengan hasil respons  kemoterapi fase induksi, penurunan Bcl-2 berhubungan tidak signifikan dengan respons kemoterapi fase induksi, hubungan tidak signifikan antara rasio Bax/Bcl2 dengan hasil BMP
