KADAR TUMOR NECROSIS FACTOR- PLASMA PADA BERBAGAI DERAJAT KEPARAHAN AKNE VULGARIS DI RSUD DR. SAIFUL ANWAR MALANG

Abstrak

 

Akne vulgaris (AV) merupakan penyakit peradangan kronis pada unit pilosebaseus. Tumor necrosis factor-α (TNF-α) merupakan sitokin proinflamasi yang terlibat dalam patogenesis inflamasi AV. Berbagai penelitian telah dilakukan untuk mengetahui hubungan TNF-α dengan derajat keparahan AV dan menunjukkan hasil yang bervariasi. Hingga saat ini, belum ada data perbandingan kadar TNF-α plasma pada individu dengan berbagai derajat keparahan AV. Penelitian ini bertujuan untuk mengukur kadar TNF-α plasma pada berbagai derajat keparahan AV dan menentukan perbedaan kadar TNF-α plasma pada berbagai derajat keparahan AV. Desain penelitian yang digunakan yaitu analitik observasional potong lintang dengan jumlah subjek 72 penderita AV yang datang ke Instalasi Rawat Jalan Kulit dan Kelamin RSUD dr. Saiful Anwar Malang dan memenuhi kriteria inklusi dan eksklusi. Subjek dikelompokkan menjadi 3 kelompok derajat keparahan AV, yaitu ringan, sedang dan berat berdasarkan klasifikasi Combined Acne Severity Scale (CASC) menurut Lehmann. Variabel yang diukur adalah kadar TNF-α plasma dengan metode ELISA. Hasil rerata kadar TNF-α plasma pada AV derajat ringan 14,72±7,97  pg/ml, AV sedang 15,39±12,13 pg/ml, dan AV berat 13,92±7,11 pg/ml. Analisis statistik rerata kadar TNF-α dengan menggunakan uji statistik nonparametrik Kruskall-Wallis menunjukkan tidak terdapat perbedaan yang bermakna (p > 0,05) pada ketiga derajat keparahan AV, namun kadarnya masih di atas rentang kadar normal. Penelitian ini menyimpulkan  bahwa kadar TNF-α plasma meningkat pada AV walaupun peningkatannya tidak sesuai dengan derajat keparahan.
